Fairfield Medical Center
Provides discharge planning and case coordination to every age group and patient population served at FMC. Facilitates the transition of care utilizing intra-disciplinary collaboration with other FMC departments, agencies, services, community resources and third party payers. Initiates discharge planning upon admission. Completes individual psycho/social assessment to determine patient care needs for a safe transition from acute care.
Performs all duties and responsibilities in accordance to the established FMC policies, procedures and guidelines.
Education: Graduate of accredited school of nursing, BSN preferred, or Bachelors Degree in Social Work
Licensure/Certification: Current Ohio RN License Required, or Social Work License Required, Specialty Certification Preferred
Experience: Three to five years of recent acute care experience required. Prior experience in case management preferred.
