To work collaboratively with women, their families, healthcare teams and the larger Cleveland community in the delivery and coordination of integrated mental health care services.
Accepts â€œwarm hand-offsâ€ from the nurse midwife or obstetrician for same-day mental health assessments.
Engages referred patients in mental health care in a sensitive and trauma-informed manner.
Determines mental health care and social needs for referred patients and coordinates linkage to appropriate services.
Provides reminder calls for patients scheduled with mental health providers and reaches out to patients who do not show for these appointments.
Assists patients in problem solving barriers to treatment (transportation, childcare, etc).
Systematically monitors patientsâ€™ treatment response, changes in clinical symptoms and treatment side effects either in person or by phone.
Maintains mental health registry to track patient follow up and clinical outcomes.
Provides brief counselling using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing or other.
Assists with scheduling on-site mental health services.
Participates in regularly scheduled caseload consultation with psychiatric consultant.
Develops and completes relapse prevention self-management plans with patients who have achieved their treatment goals and are soon to be discharged.
Experience & Knowledge:
Candidate must have experience working with socially at-risk families and patients with mental health disorders. Ideal candidate will be able to work effectively as part of a healthcare team and demonstrate an ability to liaison with community partners. Candidate must have experience with and meet clinical licensure requirements to provide brief psychological counseling.
Bachelorâ€™s degree from an accredited school of Social Work
Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board at the (LSW level)
3 years of experience working with women is strongly preferred. This position will be working with women's mental health. Experience in healthcare settings, counseling, care coordination, and knowledge of trauma-informed care practices is also strongly preferred.
